Enhanced screening and safety precautions since March 11, 2020.
It is everyone’s responsibility to follow the guidelines given by the local Public Health Authority.

Please be sure you answer truthfully to protect yourself, other patients and our staff.
If you are sick with any flu-like symptoms, fever, cough — Please isolate yourself, call Public Health or go to an assessment clinic.
